How to Change Your Address on a Passport
If your travel plans require you to leave the country, you need a passport. Keeping the information in your passport up to date is important, especially if you misplace or lose your passport. Whoever finds your passport will only be able to return it to you if your home address is listed correctly. If you move, it's easy to change the address on your passport, but if you receive your passport with incorrect data, a few extra steps are required to get it corrected.

Things You’ll Need:
  • Pencil Passport Form DS-5504 (if necessary) 2 passport photos (if necessary) Computer with Internet access
  • Pencil

Step 1
Locate your passport.
Step 2
Use an eraser to erase your out-of-date address information.
Step 3
Use a pencil to write in your current address.
Step 4
Ensure that your emergency contact information is correct.
Step 5
Use a pencil to make any changes to your emergency contact information, if necessary.

Changing Incorrect Permanent Information

Step 1
Locate form DS-5504 online.
Step 2
Complete the form: either type the information into the online form and print it out, or print out a copy of the form and handwrite your answers using black ink.
Step 3
On the second page, locate the box regarding incorrect data on your current passport. Complete this box, putting in your correct address. Double check for accuracy and legibility.
Step 4
Sign your completed form.
Step 5
Mail the completed form, your current passport, two color 2"x2" passport photos and evidence supporting your needed correction to:

If you need your passport back quickly, opt for expedited service. This requires an additional fee, but will ensure you receive your corrected passport within two to three weeks.
