Anyone under age 16 is considered a minor. Applicants aged 16 or 17 must apply for an adult passport book, although special requirements may apply.
Applications for children's passports must be made in person. Both parents or guardians must be present with the child at the time of application.
Each time a child under age 16 applies for a passport, Form DS-11 must be used. You can access the form online at the link in the reference section. Proof of U.S. citizenship, proof of parental relationship, the child's Social Security number and photo identification of both parents must be provided. A photocopy of each form of identification presented is also required. Two passport photos, taken in normal attire, must accompany the application.
Passports granted for children under age 16 are valid for five years. After expiration, the child and parents must apply in person if the child is still under age 16.
A passport book for a minor costs $60, while a passport card costs $10. Since the child needs to apply in person, a $25 execution fee is also required. For both a passport book and a passport card, the total cost is $95, including the execution fee.
