Philippines Passport Requirements

First time applicants for Philippine passports need to know that only citizens of the Philippines can obtain a Philippine passport. The Philippines recognizes a citizen of its country if you were born in the Philippines, or were born to Philippine parents. If you were born outside of the Philippines, as long as you have not been naturalized by another country, you are still eligible for a passport.

Application Form

The application form can be downloaded online at dfa.gov.ph.

Personal Appearance

You must appear in person to the Philippine Consulate where the application for the Philippines passport is being made.

Birth Certificate

A birth certificate is required.

Three Color Photographs

Three color photographs, size of 4.5 cm x 3.5 cm, with the size of the face being no less than 3 cm are required. The background of the photograph should be royal blue.

Proof of Identity

Proof of identity documents that state your full name, date and place of birth, and citizenship are required.

Other Supporting Documents

For married women, a marriage certificate is required for those who want to use the last name of their husbands.
