Original Passport
You must have a valid passport to obtain a Visa for entry to Vietnam. The date on your passport must not expire more than one month before you intend to leave VIetnam, or more than six months before you leave, if your airline requires an extra five months. Mail your original passport to the Vietnamese Embassy in the United States at:

Application Form
Your passport must be accompanied by an application form and photo. Obtain the application form by downloading it from the Vietnamese Embassy's website at vietnamembassy.us. If you have prior Visa approval, write the Visa Reference Number on the top of your application form. Complete and sign the application form, and attach one 2 inch by 2 inch photo of yourself.
Visa Fees
The fee schedule, as of 2010, for issuing new Visas is posted on the Vietnamese Embassy's website at vietnamembassy.us. The fee for a Visa good for one entry is $30. To enter more than once in a single month, the fee is $50; to enter more than once within six months the fee is $80; and to enter more than once for six to twelve months the fee is $120. Additional fees may apply, and current fees should be verified by emailing consular@vietnamembassy.us.
All fees must be paid with a money order, cashier's check or certified check made out to "The Embassy of Vietnam." If you are submitting multiple applications in one package, you may lump your fees into one money order or check.
Prepaid Return Envelope
Include a prepaid return envelope in your package. The Vietnamese Embassy prefers a stamped envelope to be returned through the United States Postal Service or a prepaid FedEx envelope. The Embassy recommends purchasing additional tracking services for your envelopes. Multiple Visas may be returned in one envelope.
