How to Replace a Lost Passport in Memphis Tennessee

Whether you're in Memphis or Mumbai, one way to protect your passport is to keep it in a colorful, waterproof and loadable container. This increases the chances of its safe return if you lose it on the trail or on water. Otherwise, make copies of it and all your travel information. Exchange the duplicates with your travel buddies and keep one in a waterproof pouch away from the original. These copies will prove useful in replacing a lost passport.

Instructions

Step 1
Fill out Form DS-64, Statement Regarding a Lost of Stolen Passport, which requires information from the copies you made, like passport number and issue date.
Step 2
Report the loss by calling the National Passport Information Center at (800) 487-2778. You will need information from Form DS-64, so keep it handy. Because you phoned in your report, you can keep the form for later submission.
Step 3
Fill out Form DS-11, Application for Passport, to apply for a new passport. In question 22, fill in your passport name, passport number and issue date, and choose "Lost." If the passport had expired, write "EXPIRED" in the "Other" box. Do not sign this application until requested by the passport agent.
Step 4
Assemble additional supporting documentation. You require evidence of U.S. Citizenship including a certified birth certificate, naturalization certificate or a consular report of birth abroad.

You also need a government-issued photo ID like a driver's license, and a photocopy of it. If your official ID is also lost, you need an identifying witness who is a U.S. citizen or permanent resident who has known you for two years or more. He must submit his own official ID and sign Form DS-71, Affidavit of Identifying Witness, in front of the agent.
Step 5
Supply two passport photos, which can be taken at a passport photo shop or Acceptance Facility with cameras. Your identical, color pictures must measure 2-inches square. You must wear ordinary street clothes plus any wigs, glasses or hearing aids, if you normally wear them. Uniforms or hats are not allowed but you can don religious attire that is used daily.
Step 6
Go to an Acceptance Facility like one of the following:

Visit in person with your Forms DS-64 and DS-11, and supporting materials. When requested by the agent, sign your form and pay any fees. If you want your passport in less than two weeks for international travel or less than four weeks for a foreign visa, pay extra for expedited processing.
