Passport Information for Children
Passports are required for individuals of all ages whose travel agenda carries them across U.S. borders. This document is also required for authorized re-entry into the country. Unlike adult passports, children's applications require special considerations for issuance.

Minor Child Passport

All children, including newborns and infants, are required to have their own passport. Passports are issued to children ages 16 and under.

Form DS-11

All children's application must include a completed Form DS-11. The form may be completed online or in person. The form must then be signed in the presence of an authorized Acceptance Facility officer.

Parental Acknowledgement

Children's passports require parental acknowledgement, and all children's applications must be completed in the presence of both parents and/or guardians. Additionally, you must show proof of citizenship, such as a U.S. birth certificate, Report of Birth Abroad, Naturalization Certificate or Certificate of Citizenship.

Passport Cards

Children are permitted to obtain a minor passport card. Like adult passport cards, these cards are only authorized for international travel that is completed by land or sea.

State Identification

Children 16 and under are required to carry a state-issued identification card or driver's license along with their passport. Newly issued applications for 16-year-olds must include a copy of this identification.
